Welcome to our practice.  We are pleased that you have chosen our office to
participate in your medical care.  Please take the time to look around our website
and familiarize yourself with Dr. Agra, Tani Olmos, NP and the practice of
nephrology.  

At your first appointment we ask that you bring a current medication list.  Please
include the strength of each medication and how many times per day you are taking
it.  If you are keeping a blood pressure log, bring that as well.  Your vitals will be
taken and your medical specialist will discuss the information that they have
received from your referring physician.  A unique plan of care will be created for you.  
Please bring the names of other doctors that you are seeing.  After your visit, your
practitioner will compose a letter detailing your condition and send it to your doctors.

Please make sure that your new patient paperwork is completed prior to arriving to
our office.  The forms are linked below and will open with Microsoft Word.  We will
be happy to send them if you are not able to download the forms.
